If you would like to register with the practice you will need to be living in our practice area. Ask the receptionist for a registration form and you will then be given further information. We ask all new patients to provide proof of identification. (e.g. passport, driving licence) and proof of address (e.g. utility bill or bank statement).
You will be offered a new patient health check with the practice nurse or the health care assistant.
Patients are registered with the practice rather than with individual doctors, however, a patient may request to be seen by a doctor of their choice.
The premises are readily accessible to the disabled, with the provision of a wheelchair ramp, handrails and disabled lavatory.
